Your record
Your goals
What you are aiming at, the baseline habit underneath it, and how the goal changed as your reading of the problem changed. The revision history is kept on purpose. A goal that was rewritten three times is the clearest evidence there is that somebody understood the problem better each time.

How a goal is written here
A goal names one baseline habit, not an outcome. Hold one deliberate pause in every meeting is a goal. Be a better speaker is a wish, and nothing can be checked against it.
Baselines are counted in days returned to rather than hours spent, for the same reason practice is counted that way on your dashboard. A habit is a days number.
When a goal is rewritten, the old wording is kept beside the new one with the date. Nothing is overwritten and nothing is deleted, so the history reads as a record of what you learned rather than a list of things you failed at.
What a goal is aimed at comes from your Diagnosis. How it is worked is the five steps.
